Guide for referrers, families and care teams
A clear referral helps us understand the reason for care, contact the right person and discuss a suitable appointment. You can request a home visit, a clinic appointment or care in a residential aged care facility.
1. Start with the details needed to arrange contact
Use the KMW referral page for the initial enquiry. Have the patient’s consent, or appropriate authority to act for them, before sharing information.
- Patient: full name, suburb and the best contact name and phone number.
- Visit preference: home, clinic or residential aged care.
- Reason: a brief description of the concern and what help is being requested.
- Access: relevant mobility, communication or support needs.
- Referrer: name, organisation, work email and phone number.
- Payment pathway: private payment or the relevant funding program, and a billing contact if different.
Keep online enquiries brief. Do not put full medical records, plan numbers or identity documents in the general enquiry form. Our team can explain what further information is needed and how to provide it. See the KMW privacy policy.
2. Provide relevant supporting information when requested
GPs and care providers can fax referrals to 08 7081 3009, or call 1800 962 113 to discuss another suitable way to send documents. Include information relevant to the referral, such as:
- The foot or lower limb concern, when it started and how it affects daily activities.
- Relevant conditions, medicines, allergies, previous treatment and any current wound-care instructions.
- Useful assessment or imaging reports, if already available.
- Who can assist with appointments, communication or transfers, where applicable.
- Funding approval, referral or invoicing information needed for the proposed care.
A focused summary is usually more useful than an unrelated full record. A routine referral enquiry should not delay assessment of an urgent foot problem.
3. Check the payment and funding arrangements
NDIS
KMW works with plan-managed and self-managed participants where podiatry is an eligible disability-related support. KMW is not a registered NDIS provider and cannot provide services paid from NDIA-managed funding. Explain the relevant disability-related needs and goals, and confirm funding and billing arrangements before booking.
Support at Home
Tell us the provider’s name and care coordinator’s contact details. Podiatry must relate to the person’s assessed needs and approved support arrangements. Let us know if a quote or provider approval is required. Equipment or footwear recommendations do not themselves confirm funding approval.
Other arrangements
If using a GP care plan, DVA or a workplace injury claim, tell us when you enquire so we can check the relevant referral and payment requirements. Private patients can also contact KMW directly. Fees and any applicable gap are confirmed before booking.
What happens next?
Our team checks the location, availability and information needed to arrange care. We may contact you or the nominated patient or carer to clarify details. The appointment is booked once the arrangements are confirmed; submitting a referral does not reserve an appointment or approve funding.
