How to Arrange Podiatry Care

  1. Make the initial enquiry. Tell us the person’s suburb, a contact number and the general type of podiatry care needed. If you are contacting us on someone else’s behalf, make sure they know and agree to the enquiry, or that you are authorised to act for them.
  2. Discuss the arrangements. Our team will check visit availability and discuss any referral, funding, fee or provider requirements before booking.
  3. Provide the necessary documents. We will explain which information is needed and how to send it. GPs and care providers can fax referrals to (08) 7081 3009.
  4. Confirm the appointment. The appointment is arranged after availability and the relevant details have been confirmed. Your podiatrist will assess the person’s needs and discuss suitable care.

Funding and Referral Arrangements

Private patients

You can usually book private podiatry without a GP referral. Ask our team to confirm the fee, payment arrangements and any private health claiming requirements before booking.

Support at Home

Podiatry may be arranged where it meets the person’s assessed needs and approved support arrangements. KMW can discuss care, quotes and relevant documentation with the person and their provider. Read about Support at Home podiatry.

NDIS

KMW works with plan-managed and self-managed participants where podiatry is an eligible disability-related support. KMW is not a registered NDIS provider and cannot provide services paid from NDIA-managed funding. Confirm funding and invoicing arrangements before booking. Read about NDIS podiatry.

DVA

Eligible Gold Card holders, and White Card holders receiving care for accepted conditions, may access clinically needed podiatry with a valid referral. Tell us you intend to use DVA arrangements when you enquire. Read about DVA podiatry.

GP care plans

A Medicare rebate may apply to eligible podiatry with a valid GP Chronic Condition Management Plan referral. Confirm the fee, any gap and claiming arrangements with our team. Read about GP care plan podiatry.

Residential aged care

Families and facility staff can enquire about visits for residents. We will discuss the facility’s arrangements and any fees or funding before care is booked. Read about residential aged care podiatry.

For GPs, Providers and Care Coordinators

Let us know if you need a quote, provider approval or clinical documentation to organise the service. With the person’s consent or other appropriate authority, we can discuss the information needed to coordinate podiatry care. A referral enquiry does not confirm funding approval or appointment availability.

For general enquiries, call 1800 962 113 or email [email protected]. Referral fax: (08) 7081 3009.

Send an Initial Professional Referral

Use this form for the details needed to begin care coordination. Please obtain the patient’s or authorised representative’s consent first. Keep clinical information brief; contact reception or use the referral fax above for supporting documents.

Referral details are used to assess and coordinate your enquiry under our Privacy Policy. Please send only the details needed for initial contact. KMW will discuss any further documentation and funding requirements with you.

    Home Visits or Clinic Appointments

Home visit availability depends on the suburb, appointment schedule and care required. Tell us where the person lives and we will check the options. Learn about home visit podiatry or find a KMW clinic.
